This section of the UB04 allows for several options for ordering charges on claim output. Users may choose to have charges print in Numerical Order (ascending Revenue Code order), Entry Order, or 0360/0490 first, then Numerical order. This last option will print the 0360/0490 (ASC) Revenue Code first and then all additional revenue codes in ascending order.
Diagnosis & Procedure Coding (66 & 74a-e)
For FL66 there are two values in addition to a blank value that can be set for this field. The default selection for these fields is Carrier. This field will look to the Insurance Carrier dictionary to see what date the carrier switched from ICD 9 to ICD 10 codes (International Classification of Diseases). If the Date of Service (DOS) being billed is on or after that date, the system will apply the ICD 10 codes. If the DOS is prior to that date, the system will apply the ICD 9 codes.
If you are working on AmkaiOffice Version 3.17x or greater, you have the option to pull CPT-4 code into fields 74 a-e, when payors require it. To activate this setting by check the box Allow Cpt code for field 74 on global or carrier-specific custom (Diagnosis and Procedure Coding section).
