SPH Analytics CAHPS Report Required Fields

Data Elements Included in the SPH Analytics CAHPS report

CMS mandates that all required fields are completed when reporting survey results.

The SPH CAHPS report includes these fields, which pull data from various areas of AmkaiOffice. It is important that you review the following table of required fields and ensure your organization is collecting information to satisfy these fields.

Before embarking on the submission process, your facility should work with your SPH Analytics representative to ensure your reporting meets all requirements.

All material enclosed in quotes in the table below comes directly from documentation produced by SPH Analytics.

 

SPH CAHPS Report Field Where the data for the field comes from in  AmkaiOffice Notes (including required format where applicable)

 PatID

Demographics > Account number  
 PatVisitID Patient's Case number. Viewable on Demographics > Cases > Case number (System assigns a case number when the appointment is saved.) "Unique ID for this visit"

ServLoclD

System populates this field based on the facility where appointment was scheduled "Service Location of Visit. If you are sending data for more than one clinic, be sure to include a separate ID number for each Location/Clinic."

CCN Number

Entered by user on Report Parameters field when pulling the report. "CMS Certification Number"
CCNName Entered by user on Report Parameters field when pulling the report. "The name associated with the CCN"

PatType

System auto-populates this field with "S" when appointment is scheduled in the ASC business entity. The patient type will always be "S" for an ASC.

ProviderID

System Administration > Dictionaries > Items > Physician Dictionary > NPI National Provider ID # for the surgeon assigned to the case

ProvFirstName

System Administration > Dictionaries > Items > Physician Dictionary > Edit Physician > First Name  

ProvMiddleName

System Administration > Dictionaries > Items > Physician Dictionary > Edit Physician > Name This field will be left blank if no middle name is entered in AmkaiOffice.

ProvLastName

System Administration > Dictionaries > Items > Physician Dictionary > Edit Physician > Name  

ProvTitle

System Administration > Dictionaries > Items > Physician Dictionary > Edit Physician > Name  

ProvSpecialtyDesc

System Administration > Dictionaries > Items > Physician Dictionary > Specialty  

ProvGender

Demographics > Gender "Provider’s gender (1=Male, 2=Female or 9=Unknown)" AmkaiOffice translates gender into appropriate number.
PatVisitDt Date System populates this date in required format based on appointment date "Date of visit /procedure(4-digit year required)"

Demographics > Last Name  

PatMiddleInitial

Demographics > Middle Name  
PatFName Demographics > First Name  

PatAddr1

Demographics > Address 1  

PatAddr2

Demographics > Address 2 Address 2 (optional)

PatCity

Demographics > City System populates City when you enter the ZIP code
PatSt Demographics > State System populates State when you enter the ZIP code

PatZip

Demographics > ZIP and Extended ZIP "Zip code (zip+4 preferred if available)"
PatPhone Demographics > Home "Area Code + Phone number, (10 digit unformatted)" AmkaiOffice populates the phone number in the required format.
CellPhone Demographics > Cell "Is the PatPhone provided a cell phone? (0=No, 1=Yes)" If both a home phone and cell phone number are entered in the Demographics screen, a "1" will appear in this field. If there is a cell phone entered, but no home phone, a "0" will appear in this field.

PatEmail

Demographics > Email Patient’s email address

PatDOB Date

Demographics > Date of birth "Date of birth (4-digit year required)"
PatGender Demographics > Home 1 = Male; 2 = Female; 9 = Unknown/Missing (UB-04, field 11 or UB-92 field 15) AmkaiOffice translates the gender entered into the right format.

Unit

System populates this field with "N/A" This field does not apply to ASCs. Only applies to hospital based outpatient departments.

CPT®

Clinical Documentation module (CDM) > Operative > Procedures > Procedure Details > Procedure   When multiple CPT codes are entered, the system populates this field on the report with all of those codes in the required format.   At this time G Codes will need to be entered into the report output manually. "CPT-4 (Current procedural terminology) are 5 digit codes, G codes are 5 character alphanumeric codes. Include all eligible codes separated by semicolons in this one field, no spaces."

 

Language Demographics > Primary Language

"0 for English, 1 for Spanish, 3 for Chinese, 4 for Russian, 5 for Vietnamese, 9 for All Other" These language codes must be entered in the Language dictionary (System Administration > Dictionaries > Items > Language) exactly as they appear above.

NumPatsServed This field is calculated by the system. "This is the total number of patients who had at least one outpatient surgery or procedure during the sample month at the ASC or HOPD. This number should reflect all patients who received outpatient care in the sample month regardless of eligibility of that surgery, or of that patient, for OAS CAHPS. CCNs which contain multiple HOPD or ASC locations should note that this value should reflect the total number of patients served across all eligible HOPD/ASC locations."
CustomFld1 Field is not used. "Custom Field 1: HealthStream is not responsible for scrubbing this data for consistency."
CustomFld2 Field is not used. "Custom Field 2: HealthStream is not responsible for scrubbing this data for consistency."
CustomFld3 Field is not used. "Custom Field 3: HealthStream is not responsible for scrubbing this data for consistency."
